APPOINTMENT

  • Appointment for Outpatient

    Process of Outpatient

    (1) To schedule an appointment, please call us or access our dedicated reservation form. Personal information input is necessary for your appointment. Kindly agree to the privacy policy on the form before applying.

    (2) After entering all required information, submit the form. Please note that attaching a passport is required for your appointment, so please prepare it in advance. The date and time are not confirmed when submitting the form.

    (3) On business days, we will contact you on the same day to confirm the reservation via email. If you make a reservation through our system, a confirmation notice will be sent to the email address you provided. If it's outside business hours, we will contact you on the next business day.

    (4) Subsequently, we will confirm the reservation date. If you need to change the reservation date, please contact us by phone or through the inquiry form.

    (5) Please visit us on the scheduled reservation date. If you anticipate being late or unable to make it on the day of the appointment, please inform us. If there are any preparatory tests required, we will contact you via LINE, phone, or email.

    (6) On the day of the appointment, please bring your passport (if available) and overseas travel insurance. For returning patients, bringing these documents is unnecessary. However, please remember to check the expiration date, and if it has expired, kindly renew it.

  • Telemedicine

    How to apply Telemedicine

    At our clinic, you can also receive online consultations with Japanese doctors. However, reservations for online consultations are only available under the following conditions, as these may affect the accuracy of the examination results:

  • Health Checkup

    From Appointment to Sending Report of health checkup

    Here are the steps for making an appointment at our clinic:

    (1)Access our appointment form
    Personal information is required for your appointment. Please agree to the privacy policy on the form before applying.

    (2)After entering all necessary details, submit the form
    Passport attachment is required for your appointment. Please prepare this in advance.
    Within 3 business days, our clinic staff will contact you through your specified method.
    In case you don't receive a response within 3 business days, please contact us.

    (3)Confirm the appointment date
    If you need to change the appointment date, contact us via phone or the inquiry form.
    Depending on the examination package, a pre-examination kit may be required. It can be sent to your company's address or picked up at our clinic.

    (4)After arrive on the appointment date.
    If you are running late or cannot make it, please inform us in advance.
    On the health examination day, refrain from consuming anything except water before coming to the clinic.

    (5)After the examination, we will send you the results in approximately 4 weeks.
    Note that some tests may cause a slight delay in sending the results, so please be patient.

  • Health Check for Workpermit

    Process

    Here are the steps for making an appointment at our clinic:

    (2)After entering all necessary details, submit the form.
    Passport attachment is required for your appointment. Please prepare this in advance.

    (3)Within 3 business days, our clinic staff will contact you through your specified method.
    In case you don't receive a response within 3 business days, please contact us.

    (4)Confirm the appointment date.
    If you need to change the appointment date, contact us via phone or the inquiry form.
    For this health examination, we do not send examination kits separately. However, if you are also scheduled for a regular health check-up or comprehensive health check-up, we will send the kit.

    (5)Arrive on the appointment date.
    If you are running late or cannot make it, please inform us in advance.
    On the health examination day, refrain from consuming anything except water before coming to the clinic.
    Bring a passport (copy or digital copy) and three 3cm x 4cm passport-sized photos.

    (6)After the examination, we will send you the results in approximately 5 business days.
    Depending on the content of the results, there might be a slight delay for verification, so please be patient.
    If you need expedited results, please inform us.
